2. Assessment & costing

We complete a needs-led assessment and provide a clear, itemised cost based on assessed staffing and support hours.

3. Transition planning

We agree a transition plan with the person, families and professionals — including bridging support where needed.

4. Mobilisation & review

Support begins, with early reviews, outcome reporting and ongoing contract liaison to confirm the placement is working.
