Making A Referral

Referring someone to us.

We aim to make referrals simple and to give you an honest, timely response — including whether we are confident we can safely meet the person’s needs.

To help us assess a referral quickly, it is helpful to share the following, where available:

  • A current needs assessment or summary of needs
  • Relevant risk assessments and positive behaviour support information
  • Any legal framework details — MCA, DoLS, CTO or Mental Health Act status
  • Funding route and the commissioning contact
  • Desired timescales and any housing or location requirements

We review the referral with you and give an early, honest indication of whether we can meet the need.

2. Needs-led assessment

We assess the person's needs, risks and outcomes, alongside the MDT, to shape the right support and staffing.

3. Plan & staff matching

We co-produce a support plan and match a staff team to the person — their needs, communication style and personality.

4. Phased transition & review

We deliver a paced transition, with bridging support where helpful, then settle the placement and review against outcomes.
